Downloadable Forms for the SuperModel contestants

Models, Guardians and Contestants, SuperModel requires that all contestants sign the two forms below to participate in the SuperModel Contest for 2014. Please download, print and sign. The medical form needs to be downloaded, printed and signed as well.

Affidavit_SuperModel13.pdf (Affidavit_SuperModel13.pdf)	All Contestants are subject to Florida law and must sign an Affidavit of Eligibility, Public Release and Liability form. All forms must be complete in order to participate.
